Death Certificates

Relationships


In order to obtain a death certificate, you must complete an application. You can only request a certificate for an immediate family member:
  • Spouse
  • Parent
  • Grandparent
  • Sibling

Required Identification


To receive a death certificate, you must have one form of primary identification or two forms of secondary identification. One of the secondary forms of identification must bear your signature.

Primary Identification


  • State-issued driver's license
  • State/city/county ID card
  • Student ID
  • Government employment badge or card
  • Prison ID
  • Military ID
  • Passport (from any country)

Secondary Identification


  • A recent utility bill
  • A recent paycheck stub
  • An employment or organizational ID
  • A photocopy of their signed Social Security card
  • A recent bank account statement
  • Public assistance applications or letters
  • Signed federal tax return
  • Public library card
  • Signed valid voter's registration card

Fees and Other Information
