Birth Certificates
Relationships
In order to obtain a birth certificate, you must complete an application. You can only request a certificate for an immediate family member:
- Spouse
- Parent
- Grandparent
- Sibling
Required Identification
To receive a birth certificate, you must have one form of primary identification or two forms of secondary identification. One of the secondary forms of identification must bear your signature and or an identifiable photo. If using supporting ID, 2 forms of supporting ID and 1 form of secondary ID are required.
Primary Identification
- Drivers license
- Federal or State ID
- Federal, State, or City Law Enforcement employment ID or employment badge accompanied by employment ID
- Offender ID issued by the Department of Criminal Justice correctional facility or institution
- Department of Homeland Security, United States Citizenship and Immigration Services issued: Employment Authorization Document, Permanent Resident Card or Travel Documents (Re-entry permit, Refugee travel permit or Advance Parole)
- Military ID
- United States Department of State issued: Border crossing card (B1 for business or pleasure or B2 medical purposes), Visa
- License to Carry License
- Pilots license
- United States passport
Secondary Identification
- Current student ID
- Any primary ID that is expired
- Signed Social Security card or Numident
- DD214 Certificate of Release
- Medicaid card or Medicare card
- Veteran Affairs card
- Medical insurance card
- Foreign passport accompanied by a Visa issued by the United States Department of State
- Foreign passport in accordance with the United States Department of State, Visa Waiver Program
- Certified birth certificate from the Department of State (FS-240, DS-1350 or FS-545)
- Private Company Employment ID
- Form I-94 accompanied by the applicants Visa or Passport
- Mexican voter registration card
- Foreign ID with identifiable photo of applicant
Supporting ID
- Utility Bill dated within the last 30 days
- Paycheck stub dated within the last 30 days
- Bank account statement dated within last 30 days
- Public Assistance applications or letters (must contain information validating the identity of the applicant and dated within the last 30 days)
- Police report of stolen ID
- Official school transcript
Fees and Other Information
