This file is a form-enabled PDF. You can fill it out on a computer and print it out or you can print it out and then fill it in by hand or typewriter.
This form is for ANYONE who thinks they may not be able to physically make it to their voting district's polling station during Early Voting or on Election Day. This includes students, military personnel, or anyone who is working or travelling away from home during the election. This also includes home-bound or sick voters.

You can still vote in the midterm even if you didn't participate in the primary.


  • Friday, September 7 - first day that the Election's Clerk can accept applications for a ballot by mail
  • Tuesday, October 9 - last day to register to vote
  • Monday, October 22 - first day of Early Voting
  • Friday, October 26 - last day to apply for a ballot by mail; application must be in the Elections Clerk's Office on this day
  • Friday, November 2 - last day of Early Voting
  • Tuesday, November 6 - last day to receive a ballot by mail
  • Tuesday, November 6 - Election Day
